Supplemental Health – Hospital Confinement Indemnity Insurance

The Supplemental Health – Hospital Confinement Indemnity Insurance plan offers benefits to assist employees and their family members with out-of-pocket expenses that occur in conjunction with health-care. The Supplemental plan covers Outpatient Surgical Procedures, Hospital Confinement, Rehabilitation, and a Waiver of Premium Benefit.

Accident Insurance

The Accident Insurance Plan provides benefits for initial care and treatment, in addition to the follow-up care you may need by covering out-of-pocket expenses such as deductibles, office visit co-payments, and transportation and lodging costs that are not covered by the Medical Plan.

Short-term & Long-term Disability (STD & LTD)

STD & LTD Insurance are provided and paid for by Infinity for every full time employee in case of disability as a result of injury or sickness.

Term Life Insurance & Accidental Death & Dismemberment (AD&D)

Term Life and AD&D Insurance are provided and paid for by Infinity for every full time employee in the event of the employee’s death or catastrophic dismemberment.

401k

Upon employment all employees are eligible to participate in Infinity’s 401k Retirement Plan. Employees may roll over assets into this Plan from a previous employer’s retirement plan or an IRA. An employee is vested in his/her own contributions at one hundred percent (100%). Infinity will match a predetermined amount of the employee contributions to the plan at the end of each plan year (December 31st). Employees will be vested in the employer’s matching contributions at a rate of 25% each year.

Vacation & Sick Leave

Employees accrue vacation leave e ach pay period in which they perform work, or are in a paid status, for the entire pay period. Vacation hours build until the maximum accrual is reached.

Holidays

Infinity provides 10 paid Holidays each calendar year to most employees. The Holidays are established prior to the beginning of each calendar year and distributed to all employees. Holiday schedules may vary according to customer requirements and locations.

Other Leave

Infinity also offers other leaves such as jury duty, bereavement, and military leave.
